Make a Claim - Instructions

By timely submitting the Claim Form, you may be eligible to receive the benefit identified in the Notice and the Settlement Agreement.

IMPORTANT: Please read the instructions below before completing the Claim Form.

You may complete and submit the Claim Form online or you may complete and submit the Claim Form by mail to the Settlement Administrator. You may only submit one Claim Form via one method of your choosing.

To be valid, your Claim Form must be complete, accurate, and timely. Your Claim Form must also include a completed attestation regarding the accuracy of the information submitted. Your Claim Form may be rejected by the Settlement Administrator if any of these conditions are not met.

If you fail to submit your Claim Form by February 5, 2025, your claim will be rejected, and you will be deemed to have waived all rights to receive a cash payment under the Settlement.

File a Claim by Mail:

Read the instructions carefully, fill out the form, sign it, then mail it so that is it received by February 5, 2025, and send it to the following address:

Remember, all Claim Forms must be submitted online or received by the Settlement Administrator no later than February 5, 2025.
